Chyrurgion taken upon oath before me William

the said County this 4th. day of May Anno Dem: 1694

The Said Informt: upon his oath now Saith that about a Eleaven of
the Clocke at night on ye. Tenth day of Aprill East past
he was Called to see one Josias Yardley< no role > whoe was than wounded
in the rest parte of his heade and he this Informt. then
Start [..] the said wounde did beleive its to be mortall and
that he doth beleive that the said wound was the [..] tativer of
his death
