Surgeon to the Second Regiment of Dragoon Guards commanded
[..] John Duke of Montague doth Certify and make Oath
[..] Arnot Gentleman Lientenant of the Troop now
[..] ewkes [..] aforesaid Commanded by Captain Toadham
Wyndham and also Pay master to the above Regiment upon
Thursday the Twentyeth Day of Aprill last past received a
Fracture of the small Bone of his right Leg near the Knee
with a violent Contusion of the Joint by which Accident the
said William Arnot< no role > in my Judgement is rendred Unable and
Incapable of Travailing to London at this time without manifest
Hagard and Danger of his Life
