Parish Church, do hereby Certifye That John Travel< no role > of the Parish
of St. Mildred & Woolchurch in the Ward of Broad Street London
upon the Lords Day Commonly called
Sunday, the Fifth day o f January imme-
diately after Divine Service and Somon, did in the Parish Church afore
Said receive the Sacramens of the Lords Supper, according to the usase of
the Church of England. In Wittness whereof we have hereunto Subscribed
our Hands the said fifth day o f January in the
Year of Our Lord One Thousand Hundred

do severally make Oath, that they do know John Travell
in the above written Certificate named and who now predent hath delivered
the same into this Court: And do farther Severally make Oaths, that they
did for the said John Travell made the Sacrument of
the Lords Supper: in the Parish-Church of St. Bannet Finch
in the said Certificate mantioned, and upon the day
and at the time in the said Certificate in that behalf certified and a preffect
and that they did see the Certificate above written Subscribed by the said
Minister and Churchwarden
And farther said [..] do
Say upon their respective Oaths, That all other matters or things in the Said
Certificate recited mentioned or expressed an true as they verely beleive
