aforesaid who being now here duly chosen Sworn and charged to inquire for our said
Lord the King when how and in what manner the said

year aforesaid at the parish and Borough aforesaid in the County aforesaid was sick and diseased
in his body of which said sickness and disease the said

of December in the year aforesaid at the parish and Borough aforesaid in the County aforesaid
did dieAnd so the Jurors aforesaid upon their oath aforesaid do say that the said John
Smith by the visitation of God died a natural death and by no violent means or
manner whatsoever In Witness whereof as well the said Coroner as the said William
Sage the foreman of the said Jurors on behalf of himself and the rest of his fellows in
their presence have to this Inquisition set their hands and seals the day year and
place first above written.
