aforesaid who being now here duly chosen sworn and charged to inquire for
our said Lord the King when how and in what manner the said

came to his death say upon their oath that the said

not being of sound mind memory and understanding but lunatic and distracted on the fourteenth day of November in the twenty ninth year
aforesaid at the Parish and Ward aforesaid in London
aforesaid one end of a certain piece of Cord of the value of one penny to and about a
certain Iron Staple then and there fixed in the Wall of a certain Bed Chamber in the Dwelling house of one

and the other end of the said Cord round
[..] his own Neck then and there did fix tie and fasten by means whereof he the said John
Cook did then and there hang strangle and suffocate himself Of which said hanging strangling and suffocation he the said

and there die And so the Jurors aforesaid upon their oath aforesaid do say that the said

understanding but lunatic and distracted in manner and by the means aforesaid did hang and kill himself Witness whereof as well
the said Coroner as the said Bilton Metcalf the Foreman of the said Jurors on behalf of himself and the rest of his Fellows in their presence have
to this Inquisition set their hands and seals the day year and place first above written
