(one whereof being of the Quorum) bearing date the Twelfth day of
November last

of Saint Brides
in the said City to the said Parish of St. Giles
in the Fields

of the said two Justices of the peace
, Appeal
to this Court against the same and humbly
pray that your worships will be pleased to
appoint a day for the hearing and Determining
of the said Appeal.
