Thomas Gladd< no role > [..] ofWood [..] Wigmore Street in the
County of Middlesex Esquire Maketh Oath and Saith
that this Deponent received a Summons to attend
as a Juror at the Session of Gaol Delivery held for the
County of Middlesex in June last. And saith that
at the time the said Session was held this Deponent was
confined to his house being severaly afflicted with Piles
and unable to attend his Duty as a Juror aforesaid
