Fields in the Liberty of Westminster and County of
Middx Gentleman maketh Oath that he this Deponent
did on Monday the Fourth day of December
Instant personally serve Mr. Empson of
White Chapel with a true Copy of the Appeal
and Order thereupon here unto annexed and did
at the same time shaw unto him the said
Appeal and Order And this Deponent further
says that the said Mr. Empson did then
acknowledge to this Deponent, that he was
Churchwarden of the parish of S: Mary White Chapel
