Middlesex To wit

These are to certify that John Whitehead< no role > one of his Majesty's
Officers of the Excise came this day before us two of his Majesty's
Justices of the Peace for the said County and took the Oaths of
Allegiance and Supremacy together with his Oath of Office
appointed and directed by an Act of Parliament made in the
twelfth year of the Reign of his late Majesty King Charles
the second. Given under our hands this 26th. day of July 1769
