Deponent did on the Twenty eighth Day of November
Last go to the Dwelling house or Habitation of Richard
Amphlet
< no role > in Birdcage Alley in the Mint in the
Borough of Southwark in the County of Surrey
and did then and there enquire for the Sd Richard
Amphlet
< no role > who was there told he was not at home
but this deponent Sayeth that he did Leave a Summons
with the Wife of the Sd Richard Amphlett< no role > as he
believes a Copy of which is hereunto annexed
to Informe the Said Richard Amphlett< no role > that John
Smith

Leonard Shoreditch and County of Middx aforesaid did intend
for the Second Day of December following being the
first Day of the Sessions of the Peace to be holden
