parish of St Giles without Cripplegate in the said
County.

Sheweth

That your Petr is rated towards the [..]
of the poor of the said parish Twelve Shillings
and Six peace P ann and forasmuch as you
petr rents or Small house of two pounds [..]
and fifteen Shillings P ann and is rated after
the rate of five pounds for the Same, Therefore
your petr thinks himself aggreived by the Said
Rate and appeales against the Same

Your petr. prays your Worpr to
releive him in the premises as
your Worps Shall Seem meet.
