summons to attend at the Guildhill
in the said City to enquire present, do &

and singular such matters
& things as or She behalf of our

the King Etc that at
the time of receiving such summons
he was very ill and was during
the illness

apply ten or a Dozen Leeches to his
Head he woud notwithstanding have
sent on apology for now attendance
but that the matter entirely slipped
his memory and therefore humbly
prays the Court to take off the fire
For such neglect
