Chairman and the rest of his Majesty's Justices of the
Peace for the County of Middlesex in their General Quarter
Session of the Peace Assembled.

These are to Certify


That the Bearer John West< no role > having exercised the Function
of A Preacher and Teacher of the Holy Gospel, at an Independent
Meeting House licensed by Law at Hammersmith in the
County of Middlesex We whose Names are underwritten
being Members of the said Meeting, do Approve and Judge of
the said John West< no role > to be a proper Person to exercise the said
Function, both with regard to his Abilities as A Preacher, &
One whose Life and Conversation is consistent therewith.

Therefore do Humbly pray that the said John West< no role >
may take the Oaths Etc. As Mentioned in the Statute
of King William and Queen MaryEntituted an Act
for Exempting their Majesty's Protestant Subjects from
the Penalties of Certain Laws.
