To the Worshipful His Majesty's Justices of the Peace for the
County of Middlesex in General Quarter Session of the Peace at the
Sessions House on Clerkenwell Green in the said County assembled.

The Humble Petition and Appeal of the
Church wardens and Overseers of the Poor of
the Parish of Saint James within the Liberty
of Westminster in the said County.

Sheweth

That by Virtue of an Order under the Hands
and Seals of J Leroux and R. Heaviside Esquires two of
this Majesty's Justices of the Peace in and for the said County
of Middlesex (one whereof being of the Quorum) bearing date
the Second day of May last John Montague< no role > was removed
and conveyed from the Parish of St. Luke in the County
aforesaid to the said Parish of Saint James as the place of
his last legal Settlement whereby your Petitioners
conceive themselves to be aggrieved.

Your Petitioners therefore humbly Appeal
to this Court against the Adjudication of the
said two Justices and their said Order and Prays
that your Worships will be pleased to appoint
a Day to hear and determine the said Appeal

And your Petitioners will ever pray Etc.
