on the Twenty Eighth day of April in the Year aforesaid at the Parish and County aforesaid Accidentally
Casually and by Misfortune fell into a Certain River Called the River Thames
there Situate And in the
Waters thereof was then and there Suffocated & Drowned of which sd Suffocation and Drowning he the said

did then and there Instantly die And so the Jurors aforesaid upon their Oath aforesaid do say that the said

Death and not otherwise
