but lunatic and distracted on the third day of April in the year aforesaid with a certain

and throat of sum the said

aforesaid one mental Wound in and upon his said
Throat of which said Mortal Wounds he the said

of April in the year aforesaid at the parish and Ward aforesaid in London aforesaid
