the Oath of the several Jurors whose Names are hereunder written and
Seals affixed
Good and lawful Men of the said Liberty, duly
chosen, who being then and there duly sworn and charged to enquire for our said Lord the
King, when, how, and by what Means the said

came to
h is Death, do upon their Oath say, that the said Samuel Vizard on the Twenty first day of
November in the Yeat aforesaid at the Parish and in the Liberty aforesaid To Wit in Coventry
Street
there being at which time Coach was passing through the said Street It so happened
that accidentally casually and by Misfortune the Pole of the said Coach did then and
there strike & beat to and against the Ground

whereof he the said
Samuel Vizard< no role >
did then and there receive one Mortal wound over the
left Eye upon the Temporal Bone, of which said Mortal wound He the the said
Samuel Vizard< no role >
did languish and languishing did live untill the Twenty fourth day of the Same month
of November in the same year an which said twenty fourth day of November in the same
Year he the said Samuel Vizard of the Mortal Wound aforesaid did die And so
the Jurors aforesaid upon their Oath aforesaid do say that the said Samuel Vizard in
manner and by the Means aforesaid accidentally casually, and by this fature came
to his Death and not otherwise And that the Pole of the said Coach was moving to the
Death of him the said Samuel Vizard and is of the Value of One Shilling but
whose Property or in whose Possession did not appear to the said Jurors In Witness
whereof as well the said Coroner as the said Jurors have to this Inquisition set
their hands and Seals the Day Year and place first above-written.
