House of Captain Berkley situate in Dover Street
in the Parish aforesaid,
within the Liberty and County aforesaid, And then and there standing upon
a Board on the outside and on the back part of the said House, It so happened
That the said

Accidentally Casually and by Misfortune fell
down off and from the said Board at the Height of two Stones unto and
upon the Stone Pavement on the Back Flat and thereby then and there
received a Mortal Wound and Fracture on the Back part of the Head of him

then and There instantly died. And so the
Jurors aforesaid, upon their Oath aforesaid, do say. That the said
