To the Worshipful his Majesty's Justices of the Peace
Assembled at the General Quarter Session of the Peace of our
Lord the King holden in and for the County of Middlesex at
Hicks-Hall in Saint John Street on Monday the fourth day
of July 1774

I Magnes Faulk< no role > of Battle Bridge in the Parish of Saint
Pancras in the said County of Middlesex Preacher or Teacher of a Congregation of
Dissenting Protestants do humbly apply to your Worships to be permitted to
take the Oaths and subscribe the Declaration according to the Statute in that
Case made and provided

We whose names are hereunder written Do humbly Certify unto your Worships
that Magnes Faulk< no role > is a fit and able person to preach the holy Gospel of God
having been a Preacher or Teacher of Dissenting Protestants at Battle Bridge
Chappel for sometime last past, We therefore most humbly request that he
may be permitted to take the oaths, take and subscribe the Declaration, and
that the name of the said Magnes Faulk< no role > may be registredaccording to the
Statute
according to the Statute in that case made and provided
