To the Worshipfull his Majestys Justices
of the peace for the County of Middlesex
at their General Sessions of the
peace assembled

Wee whose Names are hereunder Written
Church Wardens and Overseers of the Poor of the
Parish of Saint Mary White Chapel in the County
of Middlesex Do hereby Humbly Certifye that
Samuel Jones< no role > has Obeyed and performed a Certain
Order under the hands and Seals of Clifford
William Phillipps
< no role > and Richard Ricards< no role > Esquire
Two of his Majestys Justices of the peace for the
said County (whereof one is of the Quorum) bearing
date the Twenty fourth day of November last past
whereby the said Samuel Jones< no role > is adjudged the
Reputive father of a Female Bastard Child born
in the said parish of Saint Mary White Chapel
begotten on the body of Mary Clarke< no role > otherwise
Called Mary Charter< no role > .) therefore we desire he may
be Discharged from his Recognizance Given
under our hands this Tenth day of December
