To the Right Worspll. his Matys Justices of the Peace for ye County of Midds

The Humble peticon of William Cook< no role > of the parish of St.
Giles Cripplegate in the said County Labourer

West humbly Sheweth

That the Churchwardens and Overseers of the poor of the said parish did assess yor. Worspp.
poor Peticoner Seven shillings and six pence & for the whole year last past for and
towards the Releife of the poor there.

That yor. Worspps Peticoner rents but a Room in the house and is Extream poor
havind a wife and there small Children to maintain So that he is not able to pay the
said assesment and thinks himself & much aggreived there by.

Your Petr. there fore most humbly prays yor. Worspps
to be pleased [..] to order that his may be releived in the sd pr misses

And yor petr. shall over pray Etc.
