That by Warrant under the Hand and Seal of

Esquire
Alderman
of the Ward of
Cripplegate
without Dated the 20th. day of April last
your Petitioner was Appointed and Confirmed one of the
Overseers of the Poor
of the parish of St. Giles
without
Cripplegate
London
aforesaid for this present year
That your Petitioners business requiring much of
his Personal Application thereto and your petitioner
also being but in an Indifferent State of health is
unable
to perform the said Office but is William to Fine
for the same According to the Custom of the said parish
and having Applied for that purpose the Parishoners
have agreed to Accept of your Petitioners Fine and
that your Petitioner may be Discharged from the said
Office If this Honourable Court Shall think fit
Your Petitioner therefore most
Humbly prays That he may be
Discharged from the said Office
by Order of this Court
and your petitioner as in Duty bound shall
ever pray Etc
