here duly chosen sworn and charged to inquire for our said Lord the King
when how and in what manner the said

say upon their oath that the said

August in the year aforesaid being in and on board of a certain vessel lying on
the River Thames
there situate It so happened that the said

accidentally casually and by misfortune fell from and out of the said Vessell into
the said River and in and with the waters of the said River was then and
there suffocated and drowned of which said suffocation and drowning the said

did then and there dieAnd so the Jurors aforesaid upon their
oath aforesaid do say that the said

in manner and by
the means aforesaid accidentally casually and by misfortune came to his
