who being now have duly chosen sworn and charged to
inquire for our sd Lord the King when how and in what manner the said

say upon their Oath that the sd

on the fourteenth day of July in the year aforesaid being
in a certain Chamber on the third floor of the dwelling of
[..] William Ralfe situate and being in
the parish of Saint Luke within the County of Middlesex
and not being of Sound mind memory and Landing

but lunatic and distracted violently did throw and cast himself from and out of the said Window of the said Chamber
to and against the ground there by means whereof he the said

several mortal wounds and bruises in and upon the Body of him the sd

of which said
mortal wound and bruises he the said

as well at the sd Parish of St Leshey
in the County of Middlesex
as also at the sd parish of Saint Bartholomew the less
in the Ward
aforesaid in the City of London
aforesaid did languish and languish

last aforesaid in the Ward aforesaid the said

of the said mortal wound, and
bruises did die And so the Jurors aforesaid upon their Oath aforesaid do say that the said William
Crooker not being of sound mind memory and understanding but lunatic and distracted

in manner and by the means afsd did kill himself - In Witness whereof as well the
