riding on the Shafts of a Dray which was then passing along the Kings<lb></lb>

then and there accidentally, casually and by Misfortune<lb></lb>
fell to the Ground And the Near Wheel of the said Cart did then and there<lb></lb>
pass upon and over the Head of him the said Daniel Mountain By<lb></lb>
Means whereof he the said <rs type="persName" id="LMCOIC65101_n768-18">Daniel Mountain</rs>
