this 30th. day of June
1794 at the poor House of St. Anne Soho
in Row Street touching the Death
of Wm. [..] Urlwin< no role >

Wm. Devall< no role > Servt Mrs Ford no 11 Greek Street
Parish of St. Anne Soho, being sworn saith that
the Coach of Mr. North of Hampstead was passing
thro Greek Street about ½ pa [..] t 8 o.'Clock Yesterday
Eveng. [..] that the Deced (Coachman to Mr North)
was driving the Coach with his Sons to [..]
that the Coach stopped near to the House when
then Dept. lives & upon his per [..] iving a browed
to collect about the Coach, he went out to
sa [..] what was the Matter the Deced complain
much of Wind in his Stomach & began to
[..] his Wait coal This Deponent got
upon the Coach Box to the Deced & asked him
if he could get down & he replied not this
Dept. begged for assistance of the Persons
about but they did not give anythis
Dept. [..] soon left the Coast Box & went
into his House in order to bring some thing
for him but upon his Return be found
that the deced had fatten to the ground
furtherthis Dept. smith that before he left
the Box the Deced said When that he was
a Dead man further this Dept. saith not
