at the Parish of St. George Hanover
Square within the Liberty of Westmr.
in the County of Middlesex upon an
Inquisition touching the Death of
John George< no role > lying dead in the said
Parish Liberty and County.

Thomas West< no role > of Mount Street St. George Hanover Square in the
Liberty of Westminster , Surgeon and Apothecary , on this Oath
saith, That he was sent to on Thursday last between Twelve
and One o'Clock in the Morning to Lord Bulkeley's House in Stanhope Street
May Fair , to see His Lordship's Porter , and found a Wound
on the left part of his Forehead, about two inches in length
he was in a very insensible State, which Deponent apprehened
Made from a Concussion of the Brain, every Sympton attending
confirmed to established his opinion, on the same Day Thursday
in the afternoon he called again, when the operation of
Trepanning had been performed, and entertained as he did
