That the said

in the Year aforesaid at the Parish of Saint John the Evangelist
in the Liberty and County aforesaid, to wit, in Milbank Street
being driving a Cart with three Horses, and a Dray with two Horses being standing still in the said Street, it so happened
that as the Fore Horse in the Cart which was driving by the said

came up to the Fore Horse in the Dray
which was standing still, the said Fore Horse in the said Dray Neighed and immediately reared up and struck out his hind
Legs to and against the Breast and Sides of him the said

was passing on
with his said Cart and Horses, by means whereof he the said

accidentally casually and by Misfortune then
and there received a violent and Mortal Contusion on the Breast and Sides and a Fracture of Six of the Ribs of him the said

, of which said violent and mortal Contusion and Fracture he the said

at the Parish of Saint
John the Evangelist in the Liberty and County aforesaid, and also at the Parish of Saint Margaret
in the Liberty and County aforesaid
from the said Thirtieth day of August in the Year aforesaid until the Fifth day of September in the same Year did Languish
and languishing did Live on which said Fifth day of September in the Year aforesaid at the Parish of Saint Margaret in the
Liberty and County aforesaid to wit at the Westminster Hospital

otherwise, and that the Fore Horse in the said Dray was Moving to the Death of the said

aforesaid or of his Assigns. In Witness whereof as well the said Coroner as the Foreman of the Jurors aforesaid on behalf of himself
and the rest of the said Jurors have to this Inquisition set their Hands and Seals the Day Year and at the Place abovementioned
