Examination is hereunto annexed) it doth appear that the place of
the last legal Settlement of the said John

in the parish of Paddington
in the County of Middlesex
These are therefore to require you the said Constable and other Officer

in the next direct Way to the said Parish
of Paddington
and there to deliver him to some Church-
Warden, Chapel-Warden, or Overseer of the Poor of the same
to be provided for according to Law. And you the said
