THESE ARE TO CERTIFY, That at the General Quarter
Session of the Peace of our Lord the King, holden in and for
the County of Middlesex at the SESSION-HOUSE for the said County,
on Monday the Eighteenth
Day of July in the Thirty first Year of the
Reign of our Sovereign Lord GEORGE the Third, by the Grace of
God of Great Britain, France and Ireland, King, Defender of the
Faith, and so forth, between the Hours of Nine in the Morning and
Two in the Afternoon,

did personally appear in open Court, and immediately in open Court
did take, make, and subscribe the Declaration and Oath appointed to be
taken by an Act of Parliament made and passed in the Thirty-first Year
of the Reign of King George the Third, intituled, An Act, to relieve upon
Conditions, and under Restrictions, the Persons therein described from certain
Penalties and Disabilities to which Papists, or Persons professing the
Popish Religion are by Law subject. Dated the 18th Day of
July 17 91
