Fields in the County of Middlesex Gentleman maketh Oath and
Saith that he was present and did See the John Chauk Sign
Seal and in due form of Law Execute and deliver the Deed poll or Release
hereunto Annexed And that the Mark or Sign [..] Set and
Subscribed against the Seal Affixed at the foot or bottom thereof
as a Party Executing of the same is of the Proper Mark or Sign of
the said John Chauk< no role > and that the Names William Raffly and
Peter Averillo< no role > thereunto also Set and Subscribed as Witnesses
Attesting the due Execution thereof as the same now appears are
the proper hands Writing of this Deponent and the said Peter
