& for what Offence & to what Parish or Place the same<lb></lb>
belongs, & of the whole sum forfeited & by whom reced whether<lb></lb>
by the Justice or Clerk & to whom pd. over & when And<lb></lb>
transmit an accot. thereof to Petty Sessions when required<lb></lb>
by the Justices.</p>
