of June in the Year aforesaid being on The Main Stay of a Ship called The William and Jane
then lying in the River Thames
at Shadwell Dock
Stairs in the Parish and County aforesaid
It so happened That he the said

then and there accidentally casually and by misfortune fell from the said Main Stay into The Hold of the said Ship By Means whereof he the said

and there received divers Mortal Bruises in and upon the Head, Body, Legs and Arms of
him the said

Of which said Mortal Bruises he the said Alexander
Simpson then and there died And so the Jurors aforesaid upon their Oath aforesaid Do say That

in manner and by the Means aforesaid accidentally, casually
and by Misfortune Came to his Death
