in the thirty fifth year of the

and Ireland King Defender of the faith and so forth before

to inquire for our said Lord the King when how and in what manner the said

on the third day of June in the year aforesaid being on a certain stage erected
in a certain brew house belonging to Messrs Hanbury and Smith situate in the
parish of Saint Giles without Cripplegate
in the Ward of Cripplegate

aforesaid It so happened that the said

accidentally casually
and by misfortune fell from and off the said stage down to and against the Ground
and stone pavement there by means whereof the said

did then and
there receive on mortal fracture of the Scull of him the said

which said Mortal fracture he the said

as well at the said parish
of saint Giles without Cripplegate
in the ward of Cripplegate
without in London aforesaid
as also at the said Parish of Saint Bartholomew the less in the Ward of Farringdon

aforesaid did languish and languishing did live and afterwards
to wit on the same day and year last aforesaid at the parish and ward last aforesaid in
London
aforesaid the said

the Jurors aforesaid upon their oath aforesaid do say that the said

mannor and by the means aforesaid casually and by misfortune was accidentally killed
