good and lawful men of the City of London
who being now here duty chosen sworn and
charged to inquire for our said Lord the King when how and in what manner the said

came to his death say upon their Oath that the said Thomas

the eighteenth day of October in the year aforesaid being on board a certain Vessel
on the River Thames
It so happened that the said

accidentally casually and
by misfortune fell from and out of the said Vessel into the said River Thames
and in and
with the Waters of the said River was then and there suffocated and drowned of which
said suffocation and drowning he the said

but then and there die
And so the Jurors aforesaid upon their Oath aforesaid do say that the said

in manner and by the means aforesaid accidentally casually and by misfortune was suffocated drowned
