law and in what manner the said

came to his death say upon their oath that the said

[..] and Ward aforesaid in London aforesaid
called the Monument and not being of sound mind memory and understanding but lunatic and distracted did then and there violently and forcibly
cast and throw himself from and off the said Building called the Monument into a certain open place there situate called the Monument Yard
and to and
against the Stone pavement there Giving to himself then and there by such casting and throwing himself from and off the said Building called the
Monument into the said open place called the Monument Yard
several mortal wounds and bruises in and upon the head face back sides came legs and thigh of

did then and there instantly die And so the Jurors aforesaid upon there oath aforesaid
