and Seals of Philip Dept and

Justices of the peace
for the County of Middlesex
bearing date the 23d day of

, aged about forty years, Labourer was
removed form the Parish of Hampstead
in the County of Middlesex
to
the Parish of St. George
Hanover Square
in the said County as the
Place of his last legal Settlement Whereby your Petitioners
think themselves aggrieved and appeal to this Court against
the same
Therefore Humbly pray this Court will please
to appoint at time for hearing and determining
the said Appeal and that all persons concerned
may then attend.
