of ye. Parish of Hackney Gentleman is not
[..] nor hath not been Able to help himself
any Manner of way for nigh this Fortnight
either to Gett his Cloaths on or be taken out
of Bed whilest his Bed is made and by his
Intolerable Paines hath no Rest Day or night
haveing three great holes in his Thigh by
Which meanes Reduces him so weak that
his Limbs are almosst dead Downwards and
to Take him [..] out of his Bed will be Pressent
