Sheweth

That your Peticoner hath lived in the Paid Liberty these Nine
yeare and upward And hath not as yet been chargeable thereunto
Phit in regard the being very poore and [..] distracted and being
formed to lye in the Streets And the Churchwardens & overseers of
the Poore of the said liberty refuseing to allow your Peticoner any
thi [..] towards her releife & mainteynance [..] oe that he is likely to
perish unless comisterated and releived By yor. worpp Charity

ye. Petr. therefore most humbly prays
your Worspps to be pleased to allow her
such Charity towards her releife and
mainley name as unto yr. worpp Shall
