The Comissioners appointed by Her Majesty pursuant
to a state Act of Parliment (Entiluled an Act for granting
to Her Majesty severall dutys upon Coals for Building Fifty
new Churches in and about the Citys of london and Westmin [..]
&A) being Desirous to Proceed in so Pions and usefull a work
thought fitt hereby to request that you would Inform there [..]
Concerning the number and condicon of yr Inhabitants to [..]
so they may the better be Enabled to Judge what new Chr [..]
or Churches will be nessisary to be Erected in your
Parish

You are alsoe desired to Inform them of [..]
proper places for the scites of the said Church of Churches
for Church Yards and alsoe for houses for the habitations
the respective Minister and Lastly whether there beany [..]
or Chappels within ye Parish fitt to be made Parish Church [..]

All which you are desired to Signifie [..]
your hands with all Convenient speed that the Comissioner
aforesaid may proceed accordingly
